Failed Induction Of Labor And Its Associated Factors Among Women Induced At Public Hospitals In Ambo Town, West Shoa, Oromia, Ethiopia, 2025

Abstract

Background: Induction of labor refers to the artificial initiation of uterine contractions to achieve vaginal delivery. Although it plays a crucial role in reducing maternal mortality, the failure rate of induction and its associated factors have not been well studied in Ethiopia, particularly in the study area. Objective: To assess the magnitude of failed induction of labor and associated factors among induced women at Ambo town public hospitals, West Ethiopia, 2025 Methods: A facility based cross-sectional study was implemented among 261 women undergoing induction of labor at Ambo town Hospitals, from April 1st to May 30, 2025. Consecutive sampling technique was used to select 261 study participants. Pretested structured interviewer administered questionnaire and document review was used to collect data. Data were collected by Kobo tool box and exported to SPSS version 27 used for analysis. Descriptive Statistics was used to show the magnitude of failed induction of labor. Hosmer-Lemeshow and variance inflation factor (VIF) was used to test the model fitness and multi-collinearity, respectively. A Bi-variable and multivariable logistic regression analysis was employed and adjusted odds ratios (AOR) was used to identify those variables significantly associated with failed induction of labor at 95% CI and P-value <0.05. Results: The magnitude of failed induction of labor was 28.2% (95% CI: 23.0%-33.7%). The associated factors include gestational age (AOR =3.01, 95% CI: 1.52-7.89), Bishop Score (AOR = 5.14, 95% CI: 2.48–10.66), methods of induction (AOR = 3.39, 95% CI: 1.44-7.99), fetal weight (AOR = 5.06, 95% CI: 1.54–16.57) and Premature rupture of membrane (PROM) (AOR = 2.18, 95% CI: 1.03–4.64) were significantly associated with failed induction of labor. Conclusions and recommendation: The overall magnitude of failed induction of labor in the study area was higher than the national average. Gestational age, Bishop Score, methods of induction, fetal weight and pre-induction rupture of membrane was significantly associated with failed induction of labor. To address this emphasis should be placed on proper assessment of gestational age and fetal weight, improving pre-induction assessment through the Bishop score, and standardizing induction methods
