dc.description.abstract Back-ground: HIV Post-exposure prophylaxis is the only way to reduce the risk of HIV infection after potential exposure to an infectious agent and can reduce 80% of sero-conversion. HIV post exposure non-initiation was high among female rape survivors attended Woliso town public health facilities and there is no information why they did not initiate HIV PEP in study area. Objective: To identify the determinants for non-initiation of HIV post-exposure prophylaxis among female rape survivors attended public Health facilities in Woliso town , Oromia, Ethiopia, 2023. Methods: Facility-based unmatched case-control study design was employed from April 25 to May 30, 2023. A simple random sampling technique was used to select 242 (121 controls and 121 cases) female rape survivors medical records from five years data. Data were collected using data extraction checklist by reviewing the medical records of rape survivors and from key informants through in-depth interviews. All data were checked for completeness and consistency and entered into Epi-info statically software version 7.2.2.6 and exported to SPSS version 21. Descriptive statistical data were presented in form of tables and text using frequency and summery statistics such as mean, standard deviation and percentage. Data were analyzed by using a binary logistic regression model. A 95% Confidence interval with an adjusted odds ratio was computed to identify the presence and strength of association and the level of statistical significance was declared at p-value<0.05. The model goodness of fits was checked by using Hosmer and Lemeshow. Result: Being rural resident (AOR=2.34,95% CI:1.27, 4.31),when perpetrators were known to female rape survivors (AOR=4.39,95% CI:2.26,8.5), present to health facilities after 72 hours (AOR=7.84,95% CI:3.96,15.39) and no physical injury (AOR=1.98,95% CI:1.06,3.7) were found to be a determinants of HIV post-exposure prophylaxis non-initiation. Conclusion: This study revealed that being a rural resident, when perpetrators being known to the survivors and present to facilities after 72 hours, and no physical injury were determinants of HIV PEP non-initiation among rape survivors attended Woliso town public health facilities. en_US
