Abstract:
Background: A woman who nearly died but survived a complication that occurred
during pregnancy, child birth, or with 42 days of termination of pregnancy considered as
maternal near miss. Sub Saharan Africa maternal near miss tool criteria instead of WHO
diseases specific criteria which has drawback of very difficult to develop definitions
based on sign and symptoms was used.
Objective: To identify determinants of maternal near miss among women admitted for
pregnancy complications, labor and postnatal care in Gedo General Hospital.
Methods: Hospital based unmatched case control study was conducted among 248
women (83 cases and 165 controls). It was conducted from August 1
st to October 20,
2021. Data were collected using interviewer administered questionnaires and reviewing
patient’s medical records. Data were entered using Epi Info version 7.2.3.1 and exported
to Statistical Package for Social Sciences version 25 for analysis. Multiple logistic
regression was computed to control confounding variables and to identify determinants of
maternal near misses in which statistical significance was declared at p value <0.05.
Result: Family monthly income(AOR:0.11;95 CI:0.02,0.48), deciding to go health
facility as current problem identified(AOR:4.54;95 CI:1.56,13.21), time it takes to reach
near health facility(AOR:6.95(95% CI:2.29,21),history of chronic diseases
(AOR:0.01;95% CI:0.002.0.03),gravida(1-2 : AOR:0.07;95% CI:0.02,0.26);gravida 3 to
4(AOR:0.07;95% CI:0.02,0.32), and birth preparedness & complication
readiness(AOR:4.1(1.59,10.58) were factors significantly associated with maternal near
misses in this study.
Conclusion and Recommendation: The finding of this study found that multiple factors
associated with maternal near miss involving family monthly income, not deciding to go
health facility as current problem identified, time it takes to reach near health facility,
history of chronic diseases, being multipara and no birth preparedness and complication
readiness for current pregnancy. Most of them are modifiable. Hence working on these
factors is important.
